Appointments for established patients (New Patients, go to the Provider page)
Please click “Appointments” above if you would like to make an appointment.
You should receive an email confirmation of your scheduled appointment; if you do not, please verify your appointment by logging in to the appointment system and selecting the My Appointments tab.
We have a Patient Portal to enable secure sharing of your medical record. It allows us to send you labs, health records, and billing statements, all with the same security in place that you would expect when banking or shopping online.
If you do not yet have access to the Patient Portal, you can email us at firstname.lastname@example.org to setup your account for you. We welcome your questions, comments, and feedback.
PLEASE REMEMBER YOUR USERNAMES AND PASSWORDS!
It might be helpful to login to your account after you make your appointment and change your login and password to something you will remember. Your login for the Patient Portal is different from the login for making Appointments. However, you can change your login for the Patient Portal to be the same username and password as used with the appointment scheduler. After you’ve logged in to the Patient Portal, click the “My Profile” button to edit your username and password.
