Louise Boxill, ND, ARNP My Resume | My Story
Primary Care and Consultations
Born in Barbados, Louise is from a family that lives very close to the land, believes in nurturing healthy relationships, and promoting healing by natural means when possible. She chose to pursue degrees in Naturopathic Medicine and as a Nurse Practitioner because of her desire to serve, listen, teach, and coach women and families to reach their highest health goals and obtain appropriate tools to make informed choices for their own health and the future well being of their respective families and communities. Her hobbies include: dancing, singing, cooking, gardening, sewing, knitting/crochet, latch key, arts and crafts, outdoor time, family and friend gatherings, listening and making music, having a good laugh. New Patient Registration Procedure
1. Decide which Payment Option is right for you.
We accept many health insurances, but do require a Fee for Non-Covered Services. We also have a membership program, where a monthly fee pays for all of your office visits.
2. Schedule an Appointment (click on the enrollment tab).
Your New Patient Visit will last 1.5 hours for adults. Children and adolescents under 18 with non-eventful past medical histories may schedule a “Well Child Visit” (60 minutes) for their first visit.
3. Print out the Patient Registration Forms.
Please submit the completed Registration Forms with the below paperwork 48 hours prior to your appointment. We may not be able to see you if your information is not received as requested. Review the Patient Handbook for the practice’s policies and other important information.
4. Complete and return the Medical History Questionnaire (for adults) or Well Child Form for children and adolescents (please select appropriate age).
Please allow 1 hour for completion of the above questionnaire. Please note that there is a 3 day diet diary contained therein. Your completed Medical History Questionnaire must be received by the office (by snail mail, email or hand-delivered) 48 hours prior to your appointment.
Be aware, for most patients with complex or chronic illness it will take a few visits before our plan for healing is solidified.

Insurance Program Pricing:
A monthly fee for non-covered services is required to use your Insurance. This fee is $20/mo for an individual or $40/mo for a family.
Membership Pricing:
$100/mo for adults (18 and over)
$75/mo for children (under 18)
Cash Pricing:
1.5 hour New Patient Appointments: $300
60-minute Followup Appointment: $200
45-minute Followup Appointment: $150
30-minute Followup Appointment: $100
15-minute Followup Appointment: $50

Evan H. Hirsch, MD, ABIHM My Resume | My Story
Primary Care and Consultations
Doctor Evan, as he is affectionately known, is a big believer in the body’s ability to heal itself. He sees his role as a guide to the removal of obstacles that impede good health and a supporter of those processes that are deficient or need some stimulation. Dr. Hirsch is Board Certified in Family Medicine and Holistic Medicine. His hobbies include: singing, musical theater, basketball, travel, family time, dancing, mindfulness, nature, medical practice business models.

David Lerner, LAc, MTCM My Website
Functional Nutrition and Cancer support
David received his Diploma in Comprehensive Nutrition from Huntington College of Health Sciences in 2010. In 1997 David completed post-graduate training at the Beijing Cancer Institute and the Chengdu university of TCM where he trained with doctors specializing in oncology, gynecology, gastroenterology and dermatology.
Dr. Hirsch (or Dr. Boxill) is present for every appointment and collaborate in true integrated fashion with David at the end of each visit. Come see us collaborate on your case!

2. Schedule an Appointment (you will need to complete enrollment).
3. Review our Patient Handbook for more information about our practice and our policies.
4. Complete and return the Patient Registration Forms and the Functional Nutrition Intake.
You do not need to fill out the Membership or Insurance agreements unless you will also be seeing a Primary Care provider.

Stacy S. Hirsch, MESIntegrative Medicine Health Coaching
Stacy has over 15 years of experience as a consultant, leader, trainer, coach and facilitator. She uses her training in yoga, mindfulness, meditation and non-violent communication to inform her practice as an Integrative Health Coach. Stacy has a Masters in Environmental Studies with a focus on the relationship between human health and the health of our environment.
For those who wish to use their insurance, Dr. Hirsch (or Dr. Boxill) is present for every appointment and collaborate in true integrated fashion with Stacy at the end of each visit. Come see us collaborate on your case! (please see below for insurance rules)

2. Email stacy@doctorevan.com to setup an appointment.
3. Review our Patient Handbook for more information about our practice and our policies.
4. Complete and return the Patient Registration Forms, Health Coaching Agreement, and the Current and Desired States intake form.
If you are already a patient with us, you do not need to complete the Patient Registration Forms. You do not need to fill out the Membership or Insurance agreements unless you will also be seeing a Primary Care provider.
Because of the nature of Health Coaching, a minimum three month commitment is expected.
