Primary Care and Consultations

Born in Barbados, Louise is from a family that lives very close to the land, believes in nurturing healthy relationships, and promoting healing by natural means when possible. She chose to pursue degrees in Naturopathic Medicine and as a Nurse Practitioner because of her desire to serve, listen, teach, and coach women and families to reach their highest health goals and obtain appropriate tools to make informed choices for their own health and the future well being of their respective families and communities. New Patient Registration Procedure

1. Decide which Payment Option is right for you.

We offer an Insurance Program and a Membership Program. Each program requires a monthly fee. Please click on the Payment Options tab for more information. If you enroll in the Insurance Program, please make sure we accept your insurance.

2. Schedule an Appointment.

Your New Patient Visit will last 1.5 hours. Adolescents over 14 years old and under 18 with non-eventful past medical histories may schedule a “Well Child Visit” (60 minutes) for their first visit.

3. Print out the Patient Registration Forms.

Please bring the completed Registration Forms to your appointment.  We may not be able to see you if your information is not received as requested.  Review the Patient Handbook for the practice’s policies and other important information.

4. Complete and return the Medical History Questionnaire.

Please allow 1 hour for completion of the above questionnaire. Please note that there is a 3 day diet diary contained therein. Your completed Medical History Questionnaire must be received by the office (by snail mail, email or hand-delivered) 48 hours prior to your appointment or you may not be seen.

Be aware, for most patients with complex or chronic illness it will take a few visits before our plan for healing is solidified.

Primary Care and Consultations

Doctor Evan, as he is affectionately known, is a big believer in the body’s ability to heal itself. He sees his role as a guide to the removal of obstacles that impede good health and a supporter of those processes that are deficient or need some stimulation. Dr. Hirsch is Board Certified in Family Medicine and Holistic Medicine.

Hobbies: singing, musical theater, basketball, travel, family time, dancing, mindfulness, nature, business models

A message from Jan:

A word about transitions…

Gail Sheehy, PhD calls them passages. Her extensive research defines several which are distinct and almost universal in our culture. They are the doorways to new stages of life and new ways of being.

I have been an equestrienne since I was 10 years old and for the majority of my adult life I have been passionate about dressage. In that parlance we speak of transitions taking the horse and rider smoothly from one gait or movement to the next. Notably, the “down” transitions are sometimes more difficult to accomplish smoothly; they take more preparation and more “leg”.

I am preparing for a “down transition”. For several reasons related to my own health and family needs I will be leaving primary care practice at the end of January. The difficult part of this transition for me will be not seeing all of you who have trusted me with some aspect of your health care and shared with me a part of your life story.

The “up” part of the transition is that Dr. Hirsch has found and hired an amazing practitioner to continue with you on this journey. Her name is Louise Boxill; she is both a board certified Nurse Practioner AND a Doctor of Naturopathic Medicine. She is intelligent, intuitive, and broadly educated. I am comfortable entrusting the care of my patients to Louise.

I thank all of you, and especially Dr. Hirsch, for the wonderful year I have spent as a part of Hirsch Holistic Family Medicine. I wish you all … Fare Thee Well!

Functional Nutrition and Cancer support

David received his Diploma in Comprehensive Nutrition from Huntington College of Health Sciences in 2010. In 1997 David completed post-graduate training at the Beijing Cancer Institute and the Chengdu university of TCM where he trained with doctors specializing in oncology, gynecology, gastroenterology and dermatology.

4. Complete and return the Patient Registration Forms and the Functional Nutrition Intake.

You do not need to fill out the Membership or Insurance agreements unless you will also be seeing a Primary Care provider.

 



Stacy S. Hirsch, MES

Integrative Medicine Health Coaching

Stacy has over 15 years of experience as a consultant, leader, trainer, coach and facilitator. She uses her training in yoga, mindfulness, meditation and non-violent communication to inform her practice as an Integrative Health Coach. Stacy has a Masters in Environmental Studies with a focus on the relationship between human health and the health of our environment.

New Patient Registration Procedure

1. Review our Health Coaching program and read the Health Coaching Welcome Letter.

2. Email stacy@doctorevan.com to setup an appointment.

3. Review our Patient Handbook for more information about our practice and our policies.

4. Complete and return the Patient Registration Forms, Health Coaching Agreement, and the Current and Desired States intake form.

If you are already a patient with us, you do not need to complete the Patient Registration Forms. You do not need to fill out the Membership or Insurance agreements unless you will also be seeing a Primary Care provider.

Because of the nature of Health Coaching, a minimum three month commitment is expected.
